Upon your initial appointment, our doctors will perform a thorough clinical examination complete with oral cancer screening, and any necessary radiographs. The doctors will then converse, face to face, with you and inform you of the treatments options available. We’ll also be happy to provide you with an estimate on treatment time and a written estimate on the expected cost.

In order to keep your treatment costs as low as possible, payment is due at the time the treatment takes place. If you have dental insurance, as a courtesy, we will submit your forms and take your co-payment at the time of treatment. For your convenience, we accept Visa, MasterCard, American Express, and Discover. We also accept the health care credit card- CareCredit.

Please complete your Health Questionnaire and Registration Forms. You can complete these forms by printing & bringing them to your first appointment. Or for a more touch-free experience, complete on your computer with Acrobat Reader/Adobe and email the completed forms to our front desk. This will allow us to complete your registration and incorporate your information into your chart.
